For your pet's protection, all vaccines must be current. Upon admission, if your pet was vaccinated elsewhere, written proof must be presented. ALL VACCINATIONS MUST BE GIVEN BY A LICENSED VETERINARIAN. Dogs: DHPP, Lepto, Rabies, Bordetella. Cats: FVRCP and Rabies.

All dogs staying 3 days or more will be required to have a clean-up bath before going home at the following costs:

0-25lb $21.00
26-75lb $23.00
76-100lb $26.00
over 100lb $29.00

● Aggressive pets will be administered a sedative/tranquilizer by one of our doctors if deemed necessary. The fee for the sedative is at the owner's expense. We reserve the right to deny boarding for any
aggressive pet.
● Pets receiving a bath may be picked up after 1pm. Mats on your pet may interfere with a complete clean up bath.
● Pet(s) that require medication to be administered while boarding will be charged a daily medication administration fee of $9.00. This is for every day that medications are given.

I have read the above policies and give Goodlettsville Animal Hospital the authorization to treat my animal should it become ill while boarding. I agree that I will be responsible for any charges involved in such treatment. I have also read and understand Goodlettsville Animal Hospital's boarding policy and agree to abide by its requirements. THE KENNEL WILL NOT BE RESPONSIBLE FOR ANY PERSONAL BELONGINGS LEFT WITH YOUR PET.
